There are several different types of hernia, and each has it's own causes. They are classified according to their anatomical location.

Most simply stated, a hernia is caused by the weakness in the muscular wall that normally maintains organs or portions of organs in their proper place. This weakness, whether it be on the abdominal wall, diaphragm or inguinal area will lead to a breach through which a portion of intestine or other tissue can protrude. Compromise of the blood supply or pressure on the nerve supply will produce pain and the other symptoms of a hernia.
In short, a hernia is caused by muscular weakness be it from a congenial anomaly, trauma or a combination of both.
