I saw a re-run of this episode this evening and remembered this question. It's called C*A*V*E from season 7, episode 164. Under artillery fire, all the MASH personel move into a nearby cave. One of Hawkeye's patients requires additional surgery so Hawkeye and Margaret take him back to the operating room as they don't have the equipment to operate in the cave. It's during the operation that Hawkeye adjusts the lamp with the surgical clamp and leaves it hanging there. 164.
