How were international telephone dialing codes assigned and how come the US and Canada, two huge countries, share one code (1) when much smaller countries with a lot in common, such as the UK and Ireland, or Belgium, the Netherlands and Luxembourg, have different codes?

Canada may be large...but there arent actually very many people living in it...I believe i have heard we have as many people as California does
But as well...Canada (and the USA) are further divided up into smaller area codes..usually by state or province but in some cases even smaller...you actually have to dial more numbers to call the USA from Canada than you do to call Australia from Canada
