Is it true that in the early 1990's Yugoslavia dissolved into five independent countries, and if so, what are those countries?

In 1991-1992 the Socialist Federal Republic of Yugoslavia broke up into a number of smaller states. Their number and names does not have a fixed answer.
1991 - Federal Republic of Yugoslavia, Slovenia, Croatia, Former Yugoslav Republic of Macedonia (FYRM) - 4 states
1992 - Yugoslavia, Bosnia and Herzegovina, Slovenia, Croatia, FYRM - 5 states
2003 - Serbia and Montenegro, Bosnia and Herzegovina, Slovenia, Croatia, FYRM - 5 states with some border changes
2006 - Serbia, Montenegro, Bosnia and Herzegovina, Slovenia, Croatia, FYRM - 6 states
2008 - Kosovo declares its independence from Serbia - countries which recognise this would now count 7 states, those who do not recognise Kosovo would still count 6
