Marcus Aurelius ruled as emperor until 161 to 180 ce. The movie is based somewhere around the year 180 ce as the emperor dies in the movie of course.

It follows the Braveheart school of historicity, I'm afraid, and can't be said to be set in a specific year. The background events depicted in it however took place (so far as they ever did) between 180 and 192 CE (during the unhappy reign of Commodus as emperor. Commodus really was quite keen on gladiatorial combat, and liked to take part himself, even after he became sole emperor on the death of his father in 180 (though he was careful to ensure that on those occasions he was fully armed and his opponent was not).
