What causes the feeling of sleepiness or lethargy after a heavy meal?

The simple process of digestion means that your body diverts some blood flow to make it available where the food is ready to be absorbed. This reduction of flow to peripheral areas can create a sense of lethargy.

But some people experience far more severe lethargy than mere drowsiness. One culprit is sugar in the bloodstream, either from sugar in food or as the result of digestion of starches. Some experience lethargy when blood sugar gets high quickly, some when it then falls rapidly as your body goes into overdrive to restore the balance in your blood. And if it is regular and severe, it may be a symptom of a more serious underlying disease.
