Any time something stimulates nerve endings in or near the skin's surface, an itch can occur. There does not have to be an infection or sore or irritation in the skin itself. When the skin comes in contact with something outside the body, an itch can occur. In rarer cases, if the skin comes in contact with something inside the body, say below the skin, an itch can occur as well. Sometimes itching may even be a psychosomatic response brought on by something like stress, worry, or anxiety.
