The twelve days begin with the birth of the Christ child, and end on January 6th, the Feast of Epiphany. They chronicle the twelve days that the Wise Men followed the star which led them to the manger where baby Jesus lay.

The current difference between the "Old Christmas" (using the Julian calendar) and the "Modern Christmas (using the Gregorian calendar) is 13 days, not 12, and (as noted in other answers) this is not related to the "!2 Days of Christmas."
