If A-Rod can not play in 2013 for the Yankees due to injury , do the Yankees pay his salary or is there insurance to pay the salary ?

That answer is spot on - if he is found to have violated the substance abuse policy of the league again, they will cut him.
Not just for the PED's but because he is getting old and not performing to his customary high level.

Baseball contracts are guaranteed money, even if they cut him they still have to pay his contract. If he gets a PED suspension, which he'd have to fail a test for, the players association still wouldn't let the Yankees void his contract. Ryan Braun got away with a HGH failure last year and Melky Cabrera signed a 2 year 16 mil with the Blue Jays a month after he got off a 50 game. I don't even think that the other owners would agree to let them get that much off the payroll, plenty of them are paying for busted players and bum draft picks. Maybe 250 for 10 isn't that good of a business decision.
