By my own research, it appears to have happened 5 times. The occurrences are as follows:

New York Jets and Mets in 1969-70
The Jets won Super Bowl III on January 12, 1969 and the Mets won the World Series on October 16, 1969. New York held both titles for 87 days until January 11, 1970, when the Kansas City Chiefs won Super Bowl IV.

Baltimore Orioles and Colts in 1970-71
The Orioles won the World Series on October 15, 1970 and the Colts won Super Bowl V on January 17, 1971. Baltimore held both titles for 273 days until the Pittsburgh Pirates won the World Series on October 17, 1971.

Pittsburgh Steelers and Pirates in 1979-1980
The Steelers won Super Bowl XIII on January 21, 1979, the Pirates won the World Series on October 17, 1979, and the Steelers then won Super Bowl XIV on January 20, 1980. Pittsburgh held both titles for 370 days until the Philadelphia Phillies won the World Series on October 21, 1980.

New York Mets and Giants in 1986-87
The Mets won the World Series on October 27, 1986 and the Giants won Super Bowl XXI on January 25, 1987. New York held both titles for 273 days until the Minnesota Twins won the World Series on October 25, 1987.

Boston Red Sox and New England Patriots in 2004-05
The Patriots won Super Bowl XXXVIII on February 1, 2004, the Red Sox won the World Series on October 27, 2004, and the Patriots then won Super Bowl XXXIX on February 6, 2005. Boston held both titles for 364 days until the Chicago White Sox won the World Series on October 26, 2005.

    Close Sixth Time?
Although not the same city, the San Francisco Bay Area held both championships when the San Francisco 49ers and Oakland Athletics were the Super Bowl and World Series Champions for 357 days from October 28, 1989 until October 20, 1990.
