The poet is Nicholas Udall. I can't find the name of the poem, only a verse. Maybe this will help someone else find it.

I can't identify the poem but Nicholas Udall was also known as Nicholas Uvedale so he may appear under the latter surname in some reference works. He did write a play called 'Ralph Roister Doister' but he also published a book of selections from Terence with English translations 'Flowers for Latin Speaking'. He also published a translation of the 'Apophthegmata' of Erasmus.
