I have only found the following - Protective headgear was not mandated by the NCAA until 1939.

Proper footballers have not (to my knowledge) ever worn helmets.
Some have worn protective face masks - specifically to protect 'healing' injuries to the cheekbone or eye socket - most notably Gary Mabbutt and the Japanese centre back at this years World Cup.
Goalies of course occasionally wear caps but these are not for protective purposes.
