http://tv.zap2it.com/news/tvnewsdaily.html?27064 says: Osbourne's treatment will begin on July 29, and she plans to allow MTV to film her experience for the second season of 'The Osbournes.' 'People will see me having chemo. It'll be interesting,' she says. 'We need to get a different vibe going. I think I'll bring some music, introduce myself, maybe we can sing some songs and cheer the place up.'

I don't know about ratings and I don't think anyone involved are to worried about them since the show has been syndicated for the second season. This means there are millions more people that will be able to watch it. Including Canadians that don't have a satellite dish or digital TV. CTV have the rights for it in Canada and will be airing it without any bleeps. So, you can be assured those involved will be making billions in the long run, which is why Sharon Osborne was able to get a contract for a second season worth 26 millions dollars US.
