The official record is held by Nolan Ryan at 100.3 MPH, but there have been a handful of pitchers who have broken that. The Guinness Book usually only recognizes records for which they were present, monitoring both the conditions and the attempt.

Former Braves reliever Marc Wohlers has been credited with 103 and I have heard of him hitting 105, but can't find the source for it.
Hall of Famer Bob Feller claimed he hit 107.9 during an exhibition.
Minor Leaguer Steve Dalkowski is believed to have thrown as hard as 110, but he was so incredibly wild that he would both walk and strike out an average of two per inning over his career. He is the pitcher that "Nuke LaLoosh" in "Bull Durham" was based on.
