Doyle said that Holmes was based on Dr Joseph Bell, a surgeon and professor at Edinburgh University, who could sometimes deduce from a patient's appearance not only his disease but his job and where he lived. Doyle studied medicine at Edinburgh and worked as Dr Bell's outpatients' clerk.
