Having cross referenced a few sites it appears that this has NEVER happened.

Maybe this is a 'trick' question. The first FA Cup Final contested after the formation of the Football League was 1889, when PNE did the double .. so the 1888 Cup Final 'was contested by two teams from outside of the top division' (as there was no top division).
