The earliest quotation in the Oxford English Dictionary is from 'My Diary, North and South' by the British war correspondent William Howard Russell in an entry for November 3, 1861. The full phrase is 'the First Lady in the Land' and the reference was to Mary Lincoln (nee Todd).
