The earliest quotation in the Oxford English Dictionary is from 'My Diary, North and South' by the British war correspondent William Howard Russell in an entry for November 3, 1861. The full phrase is 'the First Lady in the Land' and the reference was to Mary Lincoln (nee Todd). There may have been earlier uses, and the evolution of the phrase as a kind of semi-official title was probably gradual. See http://www.theatlantic.com/issues/98apr/9804lett.htm

The first wife of a President to accept the title of 'First Lady' was 'Lemonade' Lucy Hayes, wife of 19th President Rutherford Birchard Hayes.
