When and where will the last ever Concorde flight take place?

With "fare paying passengers" it has already happened.
I understand discussions are still going on to determine where they may finally end up (ie. museums or similar and which countires other than UK) and when this is sorted I assume they will be flown there.
It is hoped by many, that at least one will be kept in operational condition so that it may take part in future air shows and have "fly pasts" at special events like royal jubilees etc.
So hopefully the answer is "NEVER"

According to a site I found that looks as thought it is most frequently visited by anorak-clad aviation enthusiasts, 17th November 2003 will be the last ever flight. The last aircraft built at Filton, G-BOAF will come home.

You've got it Linus.
If it's the site I think you mean most of them actually worked on it, so like any other 'last of' it's quite a sad time.

Teletext tonight. (10 Nov)
The supersonic jet will take off from Heathrow, fly over the Bay of Biscay and land at Filton, Bristol, on November 26.
Alpha Foxtrot - or aircraft number 216 - first flew on Friday, 20 April, 1979, and will land for the last time at 1pm on Wednesday, November 26.
However this is still being called the "last ever" Concorde flight and not the last ever.
