I could not find a definitive answer to this one, but here is the link to Life Magazine, which features the collection of Life covers and also a facility to email them with queries.

This is a "famous" sort of "trick question".
The question does not actually say "who's photograph appeared" because there isn't one.
Also there is no proof the "woman" ever existed.
The "image of a woman" that has appeared most often is .....

    The Virgin Mary?

That's a Yo from my point of view lothruin.
The Virgin Mary or Madonna (not the "Like a Virgin" variety) has appeared most often.

Wonder if Lsh will think likewise.

I thought the Virgin Mary appeared most on Time magazine's cover. Was she on the cover of Life a lot as well?

Jacqueline Kennedy Onassis with (I counted) 17 covers.

Elizabeth Taylor, I count 14 covers:

Elizabeth Taylor has appeared on the cover of Life magazine more than anyone else.

During her lifetime, Marilyn appeared on the cover of Life magazine 6 times. She has also appeared on the cover a few times (5) since her death.

I enjoy playing Trivial Pursuit™, the board game. When I do, if I miss a question, I try to pay attention to the correct answer, so that I can learn something. One of the questions I missed some time back was this one: “What woman's picture has appeared on the cover of Life magazine more than any other?” I thought of several possibilities: Marilyn Monroe, Jacqueline Kennedy Onassis… perhaps Elizabeth Taylor or Princess Diana…. all of them wrong. The answer? The woman whose picture has appeared on the cover of Life magazine more than any other is… Mary, the Mother of Jesus.

I believe this is an error. Using Life Magazine's cover search engine, which only searches through to 1972, I found absolutely nothing for the Virgin Mary. I searched keywords Mary, mother, virgin, Jesus and Christ.

In checking further, I don't believe the Virgin Mary is the answer for Time Magazine either. Using their cover search engine and keywords Mary, virgin, mother, Christ, Jesus and nativity, I found only four covers.

Most "Time" Covers - The highest number of appearances on the cover of "Time" magazine is 55. The record is held not by a supermodel, but none other than former United States President, Richard Nixon.

Keyword 'Nixon' in the search engine gave me 54 matches. I don't know what else to search to come up with anything more for the Virgin Mary.

McGruff, I understood that this (Time magazine) was a Trivial Pursuit question & answer.
I also found the same TP reference re Life magazine.

Well, here's the link for Life covers. I sure can't find any.
    http://www.life.com/Life/search/coversearch

And the link for Time. I found four. That's a far cry from Nixon's 54-55.
    http://www.time.com/time/magazine/coversearch/

    Keyword:
    Christmas - Dec. 24, 1945
    Nativity - Dec. 24, 1945, Dec. 26, 1938
    Mary - Dec. 30, 1991
    Virgin - Dec. 30, 1991, Dec. 26, 1955
    Jesus - Dec. 24, 1945
Mother, Joseph, Christ and God - no results

Found two more with keyword 'madonna'
    Dec. 24, 1951, Dec. 29, 1947
    That makes six for Time.

Searching Life again with 'madonna' I finally
found one. 'Raphael's Madonna' on the Dec. 28, 1942 cover.
