    It could refer to an exit survey, conducted by a senior employee to determine why an employee has decided to leave. From this information, the company might put into place procedures so that other employees don't leave for the same reason.

    That is also called an exit interview.

    Some US school districts have an exit exam, required for graduation from high school.
