Of course, today's medical science would be able to respond a lot more quickly to the disease and surpress it much faster, saving millions of lives in the process. One of the problems with the plague in the 14th century was that people attributed diseases to sin and divine or demonic punishment which must run its course and can't be stopped. Whereas today we know that most diseases are caused by bacteria and viruses and can be stopped or avoided altogether.

Situation Today:
The disease can be treated successfully with antibiotics but local plagues still occur in areas with poor medical facilities.
