Although not certified, Bobby Fischer (perhaps the greatest player ever) today lives a reclusive life and has made statements that clearly indicate a mental imbalance: ie. supporting the 9-11 attacks and claiming that there is a world wide Jewish conspiracy.

There was the great 19th century player, Wilhelm Steinitz, who was undefeated for 27 years before going to pieces. He challenged God to games of chess, claimed he could move chess pieces with his mind and died in a New York mental institute.

Paul Morphy, who wasn't a grandmaster because the title did not exist at the time, has turned insane after leaving chess.
Akiba Rubinstein ended his days in a asylum.
