She was killed by a bomb attached to a Japanese Hydrogen balloon.

Apparently she and the five children involved were the only civilians killed on mainland America during WW2.

Lakeview is the site of the only fatal casualties of World War II in the mainland United States due to enemy attack. On May 5, 1945, a Japanese Balloon Bomb exploded, killing five children and the wife of a local clergyman. Killed in the attack were: Sherman Shoemaker, 12; Jay Gifford, 12; Edward Engen, 13; Joan Patzke, 11; Richard Patzke, 13; and Mrs. Elsie Mitchell, 26.

On May 5, 1945, a balloon bomb killed five children and a woman, Elsie Mitchell near Lakeview, Oregon, when it exploded as they dragged it from the woods. Taking some local children on an outing, Reverend Archi Mitchell watched in horror as his wife, Elsie, and the five children, ages 11 to 13, were killed. Those six were the only known victims of the balloon bombs. However, dangers of the balloon bomb still may exist. Hundreds were never found and may still be detonated with the slightest contact.

A Japanese scientist during WWII discovered the "jet stream" of high winds in the upper atmosphere (at that time they were unknown). Balloons made of rice paper carrying explosives were launched from Japan and the jet stream carried them to the U.S. An analysis of the sand used as weights on the balloons allowed U.S. authorities to pinpoint where the explosives came from.

I've always wondered how Oregon was the only state in the US that was bombed during WWII, since Hawaii wasn't a state yet.
