    How do pills know where to go after you swallow them?

    		They don't. There are different coatings for some of them so that they dissolve in different parts of our innards (because of acidity / alkalinity / absorption requirements). Once absorbed, the chemicals travel round the whole body, but usually only have an effect in certain bits. This is one reason for having different antibiotics. You only notice the effect when the affected bit gets cured. In the case of paracetamol and such, the pain killing effect is only noticed where the pain was.
