How did the term Adam's Apple originate?

No doubt from the piece of apple that lodged in Adam's throat when he ate from the Forbidden Tree of Eden (why a piece didn't lodge in Eve's throat, since she was the instigator of the sin, I have no idea).

The instigator of the sin was the Tempter (serpent, devil, whatever you prefer to call him/it). Eve probably chewed hers properly.
