It depends how you measure it. For example, you could look at the collections of national crime statistics for various countries here on nationmaster:
http://www.nationmaster.com/cat/Crime
and you might conclude that you are most likely to be murdered in Colombia, raped in South Africa, or whatever. But bear in mind that not all countries or territories are covered here. Also, in some cases the figures for a particular country might appear relatively high precisely because their law enforcement is effective, and so they have good records and there is both an incentive and a facility for victims to report what has happened to them.

Most countries have certain areas which can be classed as 'unsafe'. Even these areas may be perfectly safe during certain hours, so avoiding such areas at specific times considerably reduces any risks. Branding a whole country as 'unsafe' because of crime statistics in a specific locality or area is misleading. I have travelled extensively and without any problem in various countries which according to media and government reports may be 'unsafe', and have also visited areas of western Europe, UK and USA which I considered very 'risky' indeed.
"Unsafe' can also have a variety of meanings, eg, disease risk, violent crime, war and terror, altitude, poor medical facilities, dangerous roads and traffic, etc, etc. Throughout the world, travellers are probably at far greater risk from disease or accident than from any other cause.

For their inhabitants, countries in South East Africa are inherently unsafe. Mozambique has a record death rate of over 25 people per 1,000 of its population. Malawi has the lowest life expectancy at 37.
Then again, if countries hit by the last tsunami are likely to be hit by another, they must be pretty "unsafe".
