Some sheet music I have mentions Franz Liszt as one candidate for this title, and http://www.devine-ent.com/shows/composers/liszt-bio.shtml seems to suggest that he at least did it frequently. The site doesn't explicitly say he was the first to do so, but the fact that it was such a sensation suggests that performances from memory were rare at that point.
