The first person to refuse their Oscar was Dudley Nichols, who got Best Writer (Screenplay) in 1935 for The Informer.

The first actor to refuse their Oscar was George C. Scott who turned down the Best Actor award for Patton: Lust For Glory in 1971.

Marlon Brando refused his Oscar for Best Actor in 1972 for The Godfather as well.
Here is an interesting site with info describing the screenwriter's dispute.
