In the World Series, never. Last year the Red Sox came back from the Yankees in the ALCS.

In 1942, in the NHL, the Toronto Maple Leafs came from a 3-0 deficit to win the Stanley Cup against the Detroit Red Wings.
In 1975, the NY Islanders, in a Stanley Cup semi-final, came back to beat the Pittsburgh Penguins from a 3-0 deficit. Ultimately, the Islanders were knocked off by the Philadelphia Flyers, who then won the Stanley Cup in a series with the Buffalo Sabres:
