Question

asks:
Does the 'guay' ending on Uruguay and Paraguay mean the same thing?

It certainly does...
From the Guaraní, the language of the local people of that area, the ending "guay" means to, or from water. "Gua", meaning to/from, the simple one letter word "y" being the Guarani for "water".

To check for the Uruguayan version, change the Paraguay to Uruguay, or you can always copy and paste this little number.
