If a couple have had three children, all of which are boys, would it be fair to say the next child would be a girl?

Unless there are any particular circumstances affecting this couple, the odds for the next birth are the same as for any other, i.e. roughly even money on a boy or a girl.
(Actually, I think male births outnumber female by something like 105 to 100).

Just ask any set of parents with four boys, such as my own.

In addition to the baby's sex being determined by his x/y chromosome,the speed of individual sperm cells might be a determining factor i.e.faster sperm fertilizing the egg tend to favour boys babies.In-vitro helps to balance the chances of either.

By "His x/y" I was referring to the father.

If I recall the statistics correctly, boys and girls are conceived in equal numbers, but because more girls survive to birth, there are more girls born than there are boys... and this trend continues throughout life... e.g. girls/women live longer than men/boys...
