Which of the current 92 English league football clubs have new grounds since the Premiership began and what are these new grounds called?

Manchester City- City of Manchester Stadium
Oxford United- Kassam? Stadium
Arsenal next season
Sunderland- Stadium of Light
Brighton- Withdean
Charlton- The New Den
Wimbledon- Milton Keynes Dons (use an old ice rink from memory)
There are more, but those are the ones I could think of.
I also know Reading moved to the Madejski Stadium, and Bristol Rovers got a permanent stadium but not sure of the years.

I'm not very knowledgeable on sport, but Chester, Southampton and Middlesborough come to my mind.

Also Leicester city moved from Filbert Street to the Walkers Stadium.

Derby County moved from the Baseball Ground to Pride Park in 1997.

Southampton the Dell to St Mary's
Middlesbrough- Riverside Stadium
Oxford is as I remembered (got a parking ticket for that memory...)
Bristol Rovers- Memorial Ground
