Never has there been an NHL or minor affiliated player die on the ice, there have been a few critically injured players but not one has died.
Travis Roy, Neven Marquat, Goalie for Chicago (sliced jugular) just to name a few who were injured on the ice.

Not in the last 5 years, but there has been one (and only) person to die due to a game-related injury.
"Bill Masterton was a center in the National Hockey League. He is the only NHL player to die as the result of injuries sustained during a game (as of the 2007-08 season).
On January 13, 1968, 4 minutes into a game against the Oakland Seals at the Met Center, Masterton was checked by Larry Cahan and Ron Harris of Oakland and fell backwards onto the ice. The force of the back of his head hitting the ice caused blood to gush from his mouth and nose.
Masterton sustained a massive brain hemorrhage that damaged the pons. The injury was so severe that doctors were unable to perform surgery, and Masterton died two days later without ever regaining consciousness.
