When a show asks you to phone in with an answer, and you can win a holiday, what happens to the money generated by the phone calls? Does the show get a certain percentage?

Too right they do! Even if it's not a premium number, they will get a rake-off from the calls. Yea, even on local call rated numbers. That's how ISPs that have 0845 numbers (in the UK) make their money. The texts in to Millionaire are £1 (I think - still haven't got a telly but see that sometimes at GingeryNutt's). Just think how many people watch, and how many will text in even with the wrong answer.

That may be true for commercial channels, but I think I've read that the BBC is not allowed to make revenue this way.
The last time I saw "Strictly Come Dancing", the revenue from phone calls went to "Children in Need".
Did the same happen for other BBC shows such as "The X-Factor"?
