This is only conjecture, but I'm assuming it originates from boxing. A boxer that is knocked down is 'saved by the bell' if the bell rings before the ref counts to ten. He has survived to fight another round.

Due to superstition many, many years ago, people were fearful of a dead person still being alive when they were buried. Therefore a string was attached to the corpses hand and attached to a bell above ground. If the person who was buried awoke then they could ring the bell and could be saved.
