Why haven't men lost their nipples through evolution and why do we have them in the first place?

We haven't lost them because there is no real reason to do so. If they were a cause of early death before reproduction took place, they would have disappeared. Evolution doesn't remove unnecessary things just because they are unnecessary. If, on the other hand, women had decided that they preferred their men without nipples, the very rare cases born like this would probably die young but very happy - and the human race would have gone into decline because many of the females would be also born nippleless, and as infant formula hadn't been invented...
The mammary glands, of which the nipples are the external outlet, are modified sweat glands in origin and possibly in the early stages of their evolution were fully functional in males. One of those things that fossils can't tell you.

Men have nipples because until a foetus is eight weeks old, it is neither male nor female. It develops nipples then "diverges" to male or female. In this sense, the female form is the template for humanity.
