What officially causes a movie to be considered B?

B stands for "budget"; a B-movie is one that's done on a far lower budget than the average movie, i.e. one done cheaper than usual.

But Blair Witch Project was done on a very small budget and it is not considered B.

miffy42 is right I believe. It refers to being the "'B' Side of a double feature. To this end they tened to have a lower budget. Remember "Star Wars" was (is) considered to be a "B" movie.
