If Bush was impeached and Cheney took over as President, would Pelosi become the Vice President or would someone be nominated and voted on?

If Bush was removed from office for any reason, medical being another possibility, Cheney would nominate someone to be the Veep. Maybe his neighbor & pal Rumsfeld. Now there's something to lose sleep over. Only if something happened to both No.43 & the Veep(say, a really bad hunting accident) would Pelosi become President. Just in case you were worrying. Read about Nixon, Agnew, and Ford to see a relevant example, in case you're too young to remember it. Ciao.

Don't forget being impeached doesn't mean you're not president anymore. Nixon was not impeached, he left before that. Andrew Johnson was and so was Bill Clinton.

I believe that being impeached means you are convicted of doing something. It takes another process to remove the president from office.

I thought that if both the president and vice president were unable to perform their duties that job of president would go to the Secretary of State, which would be Condaleeza Rice. Am I wrong?

That's a different question involving a scenario in which both the president and vice president are unable to carry out their duties. Next in line is the Speaker of the House of Representatives, Nancy Pelosi. See Question #69890.

In answer to your question, no, Pelosi would not become Vice President. Cheney would nominate a new VP and the House and Senate would have to confirm.
Here is how the Nixon impeachment played out.
During the 1973 vice-presidential vacancy, House Speaker Carl Albert was first-in-line. As the Watergate scandal made President Nixon's removal or resignation possible, Albert would have become Acting President and — under Title 3, Section 19(c) of U.S. Code — would have been able to "act as President until the expiration of then current Presidential term." Albert openly questioned whether it was appropriate for him, a Democrat, to assume the powers and duties of the presidency when there was a public mandate for the presidency to be held by a Republican. Albert announced that should he need to assume the presidential powers and duties, he would do so only as a caretaker. Presumably, Acting President Albert would have appointed a Republican Vice President because, upon the confirmation of that individual, Albert's term as Acting President would have immediately ended and that person would become President. However, with the nomination and confirmation of Gerald Ford to the Vice Presidency, these series of events were never tested. Albert again became first-in-line during the first four months of Ford's presidency, before the confirmation of Vice President Nelson Rockefeller.

Cheney would choose his own VP. Pelosi would only become President if...and only if...the President and Vice President were to leave their respective offices near simultaneously with no successors appointed (i.e., die at the same time). If the VP and the President died at the same time, this could result of something more catastrophic than Ms. Pelosi could survive herself...unless she wasn't present, which would open up a whole can of worms.
