What is the most populous river island of the world?

Maybe I should have said "littoral" island or "fluvial" island. I am not sure if Manhattan Island qualifies in respect of this.

Manhattan isn't a true river island. Yes, the Hudson River is on one side but the misnamed East River on the other side is not a river, it's a tidal strait. And besides, Ile de Montreal has about 300,000 more people than Manhattan.
