Felix Mendelssohn-Bartholdy was born simply Jacob Felix Mendelssohn. But the answer you're probably looking for is that of his contemporary, Giacomo Mayerbeer, whose real name was Jacob Liebmann Beer. Jaques Offenbach would have also been known among the Jewish community in which he grew up as Jacob, and his father had changed his name to Offenbach.
