Another clue: The book received rave reviews from organisations such as The Daily Telegraph, The Times etc.
According to the personal blurb on the inside cover he must have been in prison while writing this particular book as he was sentenced in 2001 and this was published in 2002.

And I think you're using the wrong logic of "insane" to get to the answer. I mean, yes, I think there are several character in this novel who are insane, but to me, the whole concept of the novel is the insane part. :)
