I feared lives would be lost!. Matthew is speaking to Charles, however Charles is staring at Carrie.
The Boatman . Charles decides to crash at Tom's castle with his friends, until he learns that Carrie is going to stay at the hotel. He then proceeds to spend the rest of the evening skulking at 'the Boatman' with her.
Father Gerald. Father Gerald (played masterfully by Rowan Atkinson) is a friend of the family and offers to conduct the ceremony - with hilarious results!
Her turquoise knickers show as she walks down the aisle. The bow at the back of the dress falls off whilst she and Charles are running to get to the Church on time. Scarlet is blissfully ignorant of this fact as she confidently marches down the aisle.

I had not been totally satisfied with my first attempt at this question (backed up by the small number of people who got this answer right). I have now amended it. Apologies to previous players who may have got this one right if it had been worded this way from the start.

Gareth dies suddenly. Gareth collapses during Hamish's speech.

Marry the person that he loves with his whole heart. David interrupts the ceremony because he has thought of a third option for Charles (referring to a conversation they had earlier).

It's wedding number one. Charles and Scarlett are late. Unfortunately, Charles is the best man and he's forgotten something of importance... the rings. Sheep. Later, when we see Angus and Laura (the groom and his bride) open the door of their car, a sheep runs out.
They run.. They're running because they are late. They can't drive because their car has been jacked. As they're running, the bow on Scarlett's dress falls off leaving a huge hole in the dress where it had been. However, they're so late that Scarlett tells Charlie to "leave it".
At this wedding, Charles is seated at a table with many ex-girlfriends. Soon after he finds out what the seating is, he remarks that the only thing that could make this wedding worse would be to see his ex-girlfriend Henrietta. Duckface. It's possible that Fiona's dislike of Henrietta has more to do with her love for Charlie than with Henrietta's resemblance to a duck.
Hamish. Carrie says that her fiancee's name is Hamish when she introduces him to Charles at the second wedding. Charles wouldn't have been so unhappy, had he known that Carrie and Hamish wouldn't stay married for very long after their wedding.
Chester. At first Chester tells Scarlett that his name is Rhett, but that's just a joke, because Scarlett says that she was named after Scarlett O'Hara, a character in 'Gone With the Wind.' Rhett is another character from 'Gone With the Wind'.
