eyes. Bane, who was taken over by Smith in the second movie, grabs a lose wire and shoves it into Neo's eyes. When this happens, Neo's eyes are burnt and he was currently blinded.

yes. The Deus ex Machina has this discussion with Neo about Smith. Neo: "The program Smith has grown beyond your control. You can't stop him, but I can." Deus ex Machina: "We don't need you!" ...(time goes by) Deus ex Machina: "And if you fail?" Neo: "I won't." Then all these plugs just find their way to Neo. All the plugs go into everyone of Neo's plugs. Which are all over his body. Including his head.

There is peace. Neo asked the machines if he could stop Smith, that there would be peace. The machines accepted because they realized that they couldn't beat Smith alone.
