Bilbo was extremely old by the time of the War of the Ring, with his life being unnaturally extended by posession of the One Ring. On its destruction, he began to feel his age, and eventually crossed the Sundering Seas at the age of one hundred and thirty three (finally beating the Old Took's record as the longest-living hobbit.)

Gandalf delivered the message to Aragorn after they met again in Fangorn. It was a sign that Aragorn could expect the help of the Dunedain in the War of the Ring, and that he must journey along the Paths of the Dead.
