1. When Jem and Scout went to church with Calpurnia, how much money did Reverend Sikes want to collect?

Answer: Ten dollars

This very much reminds me of a scene in the movie "A Time to Kill". If you haven't seen it you really should watch it. The Reverend in that movie, however, has slightly ulterior motives.
2. How did the Reverend finally get the money?

Answer: He "sweated it out" of the congregation.

Whereas guilt was certainly a factor, and he did call out one name, Scout specifically says "The church was becoming stuffy, and it occurred to me that Reverend Sykes intended to sweat the amount due out of his flock." This was not in the movie, for which all of the actors were probably thankful.
3. What is Zeebo's occupation?

Answer: Garbage collector

After Atticus shot the mad dog, Zeebo was the one who came by to get the body. His name was mentioned twice - first when they were waiting for him and then when he actually arrived and scooped up the corpse. Then he sprinkled something from a gallon jug over the spot. (If you said "Church choir director", it's probably because he lined out the hymns.

He did this because he could read; however, it was not an occupation.)
4. Fill in the blank in Scout's narration. I'm sorry if this was hard. I realize it doesn't make much sense (to us OR Scout) but this is exactly why this stuck in my head even after I had only read it once. This was supposed to be fill-in-the-blank but it wouldn't let me and aren't you glad? 8-)
5. Why did Calpurnia come to the courtroom?

Answer: To tell Atticus the children had gone off.

She gave him a note that said the children hadn't turned up since noon, which prompted Mr. Underwood to point out that they were in the colored balcony and had been there since precisely 1:18. (Aren't you glad I didn't make the time a question?) Since he had the exact time I think he wrote it down and just sat and waited 'til someone looked for them. Typical newspaper man.
6. Who or what did Atticus find in the kitchen the morning after the trial?

Answer: Food from many of the people in town.

While it is not specified, we can assume that the food is from people in Reverend Syke's church. It's too bad towns were (and some still are) divided that way, but people didn't look at guilt or innocence.
Atticus was given no money for his work, the only offering given was to help Mrs. Robinson.
7. What was in the bottle in the brown paper sack that Mr. Raymond was drinking out of?

Answer: Coca-Cola

(In the book it's spelled "Coca-Cola".) When Dill got sick of the trial and went outside, Scout followed in time to see him take a sip. Scout was shocked (and probably slightly jealous), but it turned out to be plain Coke. Mr. Raymond didn't care what people thought of him, and didn't think what he looked like should be the thing to change people's opinions.
8. What did Atticus say after Bob Ewell spit on and threatened him?

Answer: "I wish Bob Ewell wouldn't chew tobacco."

Scout and Jem wanted him to do something, but Atticus knew it wouldn't do any good. He did remark "What am I supposed to do, put him under a peace bond?"
He also said that if it saved Mayella Ewell one more beating it'd be worth it.
Scout asked Jem why he wouldn't carry a gun, Jem replied that Atticus thought carrying a gun was an invitation for someone to shoot you.
9. When Miss Maudie was talking to Scout she said the Bible in the hand of one man was worse than a what in the hand of Scout's father?

Answer: A whiskey bottle

She said this because what the older Radleys had done to Boo. Saying it's better to have a good man drink shows exactly what she thought of both Mr. Radleys.

I took all the other names from women mentioned when Jem and Scout went to Calpurnia's church.
Lula was the woman who didn't want white children there.
Annette was having the Missionary Society meeting at her house.
Constance had better watch out, as she was in grave danger for quarreling with her neighbors, and had put up the first and only spite fence. The other three are barely mentioned but you hopefully knew the answer off the top of your head.
11. What strange food did Atticus want to eat in the living room?

Answer: Pickled pig's knuckles

When he went into the kitchen where all the food was he saw a jar of pickled pigs knuckles. He picked 'em up, smiled, and wondered aloud if Auntie would let him eat them in the living room.
12. Which of these items did Jem and Scout not find in the tree?

Answer: A first-prize ribbon for winning a race

In all there were: gum, a box covered with gum-wrapper tinfoil that held 2 Indian-head pennies, a gray ball of twine, carved soap figures, more gum, a spelling medal and a pocket watch that wouldn't run on a chain with an aluminum knife. The hole was filled up by Mr. Nathan the day before Scout and Jem were going to put their letter in.
13. What did Mrs. Merriweather call out when it was time for Scout to march on stage?

Answer: Po-ork

Scout was a ham, however Mrs. Merriweather called out Po-ork. This was mentioned twice, first when Jem was helping her practice and last during the actual program.
14. Why did Atticus think it is a sin to kill a mockingbird?

Answer: Because they do nothing but try to please people with their singing.

Interestingly enough, it was Mrs. Maudie who explained why Atticus said they could "shoot all the bluejays you want, if you can hit 'em, but remember it's a sin to kill a mockingbird."
15. What was the book Atticus was reading at the end of the story?

Answer: The Gray Ghost

While the Bible or even "Faust" may have been appropriate, Atticus was reading "The Gray Ghost", one of Jem's books. "Tom Swifts" were a bunch of great books Jem, Scout and Dill swapped. Dill had offered to swap Jem "The Gray Ghost" when he dared him to touch the Radley house. Of course, he did, and thus he has the book in his room.
