Bella decided to move to Forks to stay with her father, because she knew her mother was unhappy about the fact she had to stay home and look after her, when she really wanted to travel with her husband. Even though she didn't really want to move to Forks with her dad, she felt she had to for her mother's sake.

In the book it doesn't state who turned Alice into a vampire. It only tells us that Alice had a vision of herself and Jasper finding the Cullens and then they went and found them. So the origin of Alice is unknown.
