The movie opens with Paul, Sarah, and Travis bringing the body of Sarah's father, Bud, out into the woods. Afflicted with a horrible disease, they wheel him out into the forest, place him on the ground, cover his head with a pillow, and fire a shot into it to put him out of his misery. Travis watches as his father pushes the body into a shallow grave, douses it in gasoline, and lights it ablaze.

The black smoke that plumes off the body disperses into the air. That evening, Sarah regrets letting Travis be involved in this act, thinking it to be psychologically devastating. Travis has nightmares that night.

Travis is awakened from his nightmare by his mother because someone has gotten into the house. The family heads down the hall to the red door separating their house from their workroom just as someone tries to break through it, and Paul is able to fire off a warning shot, scaring the intruder back into the room behind him.

The man drops his weapons and complies with Paul, insisting that he's alone and there as a mistake, but Paul knocks him out anyways, just to be safe. A quick search outside proves this to be the truth.

Answer: The city

The following morning, Paul wheels the mysterious stranger out into the woods and ties him up, taping his mouth shut and placing a bag over his head. They all leave him there, even after he regains consciousness, to ensure that he doesn't show any signs of infection, even if this takes another night to prove. Paul ends up talking to him the following morning and finds out that the man, Will, left his wife and son behind at an abandoned house about fifty miles away in search of water and supplies.

They left the city when people started getting sick, but their car broke down. He was desperate.

Answer: Sarah's

Will offers food-- he has two goats and six chickens and they're healthy-- in exchange for some clean water. And while Paul is hesitant to trust Will, he gives him a drink of water and goes inside to discuss the possibilities with his family. Sarah suggests that if Will is telling the truth, they bring his family back so that they can better defend their home, so Paul offers to drive out to collect them (with Will) and their supplies so that they can all shack up together. Paul unties Will and they hop into the truck-- Paul behind the wheel and Will in the truck bed.

It doesn't take much driving in the woods before something bad happens. As Paul drives down a narrow country road, shots are fired at the vehicle and he swerves off the trail, rolling down into the trees. He quickly takes position under his vehicle with his gun, spotting one of the assailants and shooting him several times. Will is able to subdue the other with his fists. Shortly after, the two of them clean themselves up and bury the bodies. Fortunately, they all end up making it in due time, and after a while, Paul returns to the house with Will and his wife, Kim, and young son, Andrew.

Upon returning to the house, Paul gives Will and Kim some ground rules-- the red door to the workroom and the door before that stay locked at all times unless they leave (for the bathroom, for instance), and if they leave they do so in groups of two at the least. Besides that, they eat meals together and perform their necessary chores to make sure that everything gets done.
And before long, things become somewhat routine. Everyone gets along, even in the tight quarters, as they go through their day-to-day. Travis, however, continues to have nightmares, often about infection.

Answer: A dog

One day, Travis' dog runs into the woods chasing something that Travis never sees before disappearing entirely. There's little they can do though; once the dog goes far enough, there's no catching up. Paul is forced to send them all back without the dog. That night, Travis awakens to find Will's young son, Andrew, sleeping on the floor in another room, having sleepwalked there in the dark.

After taking him back to his room, Travis hears something downstairs at the back door, and when he checks to make sure everything is okay, he finds that the red door is ajar.

When everyone wakes up to check out who's intruded, it turns out to have been the dog, now infected, dying on the floor.

Answer: Andrew

After Paul and Will put the dog out of its misery, burn it, and bury it, they convene with the rest of the household in the living room. Travis tells everyone that Andrew was asleep in a different room and he brought him back before finding the door open, and perhaps it was actually Andrew who opened it. Will, of course, says that his son couldn't have possibly done so-- he's too young to reach the locks. Before this argument can escalate, Paul says that as a precaution they should stay in their separate rooms for a few days, just to ensure no one is infected.

Answer: False

Travis continues to experience terrible nightmares in which he keeps waking up to find himself infected, but he ends up awakening to find the opposite. In the night, while his parents sleep, he leaves the bedroom to hear Kim and Will discuss leaving the house altogether, and when he wakes Paul and Sarah to tell them, they mobilize, donning their protective masks and grabbing their weapons.

They don't have many options; they have to act to avoid risks. Paul heads to the other room to see if any of them are sick, but this leads to a situation where he ends up on the wrong end of a gun.

It ends up becoming a bit of a fight, but Paul and Sarah manage to subdue their guests and get them outside.

Answer: Travis

Paul and Sarah continue to aim their weapons at Will, but he fights back in an attempt to protect his family. Will knocks the gun from Paul's hands and begins beating him with a rock, but Sarah, still wielding a rifle, manages to shoot Will in the back. Kim and Andrew run into the woods to try and escape on their own, but Paul grabs the rifle from his wife and shoots and kills Andrew. Kim, who begs to be killed as well, is also shot. Paul, Sarah, and Travis, alone once again, can only look on. Shortly after, Travis begins throwing up blood and showing signs of infection.

It isn't long before he dies in bed. Paul and Sarah soon start to show symptoms as well. There's nothing they can do but wait.
