Answer: Black triangle

In the opening scene, a man and woman flee from a house with a black triangle painted on the front door. While the man gets away, the woman is shot in the back and two men finish the job by dousing her in gasoline and setting her on fire in the yard. The two of them realize that the escaping man really has nowhere to go.
The man actually makes it to a forest road where Daniel, a local deputy, has parked his car. A quick look leads Daniel to presume the man needs medical attention, so it's off they go with the man in his back seat.
2. The movie takes place at Marsh County Memorial Hospital, a location that is quiet for what reason?

Answer: The building is closing down

Marsh County Memorial Hospital was subject to a fire a short time before the movie, so the remaining staff are there to oversee minimal emergencies and pack up any remaining equipment and files for transfer to another hospital. Otherwise, save for a crew of four, the building is nearly abandoned.
When Daniel arrives, the patient is placed on a stretcher and wheeled off to another room to be sedated by a doctor and Daniel's ex-wife, Allison, who just happens to be a nurse at the very hospital.
3. Before long, the hospital is encircled by cultists wearing what colour robes?

Answer: White

In a short time, nurse Beverly kills an existing patient with a pair of scissors and Daniel walks in on her to find the bloody scene. It's even more gruesome when she turns around to show that she's cut off her own face, claiming that it's not hers and she needs help.

When she advances, Daniel shoots and kills her, fearing an attack. Daniel shoots her when she moves to attack. In a case of what the hospital staff believes is a reaction to trauma, Daniel experiences a seizure soon after in which he has a vision of visceral gore and another world. Fortunately for Daniel, State Trooper Mitchell arrives on the scene to support shortly thereafter on account of the bodies found at a nearby farmhouse. What none of them realize are the cultists, garbed in white robes, amassing outside.

Daniel, knowing he needs to call in the bodies, tries to get through on the hospital phone and his patrol vehicle but finds that both methods aren't getting through; the signal is out. In his car though, he sees a man in a white cultist outfit watching him and moves to approach.

The lights go out though, and the cultist advances on him with a blade, getting the upper hand and stabbing him shallowly in the chest. Daniel is ultimately able to get back into the hospital for immediate medical attention, but the cultists increase in number until they've completely surrounded the building. None of them realize that Beverly's body is still moving in the other room, and a tentacled creature is trying to get out of it.

After Daniel is stitched up, the man he brought in, James, awakens in a nearby room and begins screaming. He and Mitchell head over to find that a large creature, formerly Beverly, has come in in an attempt to attack and claim James, still handcuffed to the bed. Daniel is quickly able to free him from the bed by kicking one of the bars off of it and they lock the Beverly creature into the room. As this happens, the two men following James from the farmhouse arrive at the building with their guns drawn leading James to take a pregnant patient, Maggie, as a hostage at scalpel-point. James lashes out at Dr. Powell, stabbing him in the neck, while in the ensuing chaos, State Trooper Mitchell is dragged out of the lobby by Beverly, who's escaped the nearby room.

He's killed by the creature, but not before Daniel and the two others find the grisly scene and put an end to it down another corridor.

The two men following James - Vincent and Simon - quickly explain that they have no choice but to burn the whole place down. They at least burn the body of the creature they killed and toss it out to the cultists on a stretcher. Unfortunately, everyone else seems to be a hostage to the two new men, and that only makes things difficult for pregnant girl Maggie. Daniel convinces the men to join him in going out to his car (which was moved out of the parking lot) to grab his shotgun from his front seat while Allison searches for supplies for Maggie's upcoming delivery. Neither of these things goes too well.

While the guys outside are quickly surrounded once more by the cultists, Allison is attacked in a supply room by Dr. Powell, who's still alive as it would seem.
7. Who, as it is revealed, is the leader of the cult?

Answer: Dr. Powell

Daniel hears a ringing phone in one of the hospital offices and picks it up to find Dr. Powell on the other end, calling from the morgue. He reveals that he's behind much of the goings-on at the hospital as Vincent finds photos and notes proving that the doctor is behind the cult.

The photos lead them to speak to James who only tells them that the Doctor is behind the cult, which they already know. What he was behind though was much bigger than devil worship. James tries to convince them to leave, but he's pulled along with the others instead. Kim leads them via walkie-talkie as they head to the morgue... and quickly lose contact.
8. Where is the morgue?

Answer: In the basement

The guys head to the basement to search for the morgue, but on the way through the burned-out part of the building they find a staircase that isn't supposed to exist in the building. It's not what they're looking for. It is, however, a cult headquarters, and while they're down there they're attacked and James is killed. Allison, as it turns out, certainly is in the morgue. Dr. Powell tells her that he was driven by a need to bring back that which was lost to him and that he's experiencing his transformation with full consciousness, much to his delight and intrigue.

He admits that his creations in the basement with them created the fire that burned down the hospital. But none of that matters since, tonight, he plans to call his daughter back from the abyss.

He reveals that he's placed something inside of Allison, and sure enough, something writes inside her now-pregnant belly.
9. Who is the father of Maggie's baby?

Answer: Dr. Powell

Maggie starts to go into labour and Kim is forced to act, even though she has no experience in the matter. She realizes she needs to perform a C-section, but everything comes to a head. The cultists enters the building and Kim runs and hides. Maggie, meanwhile, rises and kills her grandfather, claiming that the baby within her is Dr. Powell's and that she is part of the cult as well.

She wanders off while Kim, realizing that there's no escape, hides away in a closet with the walkie-talkie. In the basement, Daniel finally finds the morgue and Allison splayed on a table inside.

It's too late for her though; the creature placed inside of her has already lashed out with its tentacles. Daniel has no choice but to kill her and put the creature out of its misery.
10. Are there any survivors who escape the hospital?

Answer: Yes

The battle comes to a head when Maggie attacks Daniel and returns to Dr. Powell's side. The doctor is able to complete the final incantation and kill Maggie, giving birth to the resurrected daughter within her. Vincent and Simon fight it, setting it ablaze (at the cost of Vincent's life) while Daniel attacks the man responsible, striking him with an axe while he gazes into the other planes. A single push sends them both tumbling through the void he opened into the abyss. It seals behind them.
Simon escapes Powell's summoned daughter as the secret basement seals shut behind him, emerging into a hospital corridor almost as though through a portal. Daylight emerges through the windows as he finds the bloody remains of Ben in the office. Kim, however, manages to have survived hiding in her closet. The two of them leave together, safe at last.
On the other side, Daniel and Allison reunite, trapped in the void forever. An obsidian pyramid floats overhead as they grab for each others' hands.
