1. What did the whole Benjamite war start over?

Answer: A deceased woman

The correct answer is a deceased woman. A Levite's concubine had run away from him back to her father's house, and he went and got her to bring her back. They stayed overnight in a Benjamite town called Gibeah, and the men of the city assaulted the house during the night seeking the Levite himself.

He gave them his runaway concubine instead, and the men abused her all night till they killed her. Her "husband" then called up all Israel to come deal with the atrocity. The story is found in Judges 19.
2. All Israel then made an oath to fight against Benjamin. What did they say they would do to anyone who did not go up with them to fight against Benjamin?

Answer: He would be put to death

As it says in Judges 21:5, anyone who did not go up with Israel to fight would be "put to death!" No wonder they all went up "as one man" to fight Benjamin (Judges 20:7)! Of course, someone didn't go up with them, but that is matter for another question.
3. The newly formed Israelite army went against Benjamin and demanded what specific thing of the Benjamites?

Answer: That they give up the guilty men

According to Judges 20:12-13, the Israelites demanded that the Benjamites give over the guilty men so that they could be executed. The Benjamites refused (Judges 20:13) and formed their own army to fight against Israel (Judges 20:14).

Answer: False

Actually, they didn't: according to Judges 20:18, they simply asked who should lead the battle, and God's answer was "Judah", as it nearly always was. According to Genesis 49:10, the scepter and the lawgiver would not depart from the tribe of Judah till Shiloh (or the Messiah) came. Basically, God answered their question as He always did: Judah should be leading the battle.

They finally did get around to asking God whether they should be fighting or not on the second day of the war (Judges 20:23).
5. The Benjamites won the first part of the war but were eventually defeated in the end.

Answer: True

The first day, the Benjamites took out 22,000 Israelite men, and another 18,000 the second day, according to Judges 20:21,25. It was not until the third day that the tide turned, and the Israelite side began to win the war.
6. What problem did the Israelite men run into after the whole war was over?

Answer: There were only 600 Benjamite men left out of the whole tribe

The Israelite men killed all the Benjamites, including women and children, except for 600 Benjamite men who managed to hide out at the rock of Rimmon in the wilderness (Judges 20:47). They then went up to the house of God and mourned the loss of the tribe of Benjamin (Judges 21:2-3). It was a little late, in my opinion, but at least they were somewhat sorry!
7. Which Israelite city did not go up with the rest of Israel to fight against Benjamin?

Answer: Jabesh-gilead

Jabesh-gilead did not go up to fight with the rest of Israel, and the men of Israel went up and murdered all of them except for the virgin young women. They took the young women captive and gave them to the remaining Benjamites instead of killing them. Unfortunately there were only 400 women from Jabesh-gilead and 600 Benjamite men (Judges 21:5-14).
8. The Israelite men had sworn that none of them would give a wife to Benjamin.

Answer: True

In the heat of their zeal, they actually did swear this, though they repented of it later (Judges 21:1). They said that any man that gave a wife to Benjamin would be "cursed!" (Judge 21:18)
9. What happened yearly at Shiloh that the Israelite men told the Benjamite men about?

Answer: There was a feast to the Lord

It was a feast to the Lord. The young women traditionally came out and danced in the vineyards at this time (Judges 21:19).

The elders of Israel found a way to get around their own "curse." They had the Benjamite men steal girls so that the fathers would not have to be guilty of "giving" them to the Benjamites (Judges 21:19-23).
