1. What "offense" did Cartman commit?

Answer: ate the skins off every piece of chicken

The boys were all waiting at Stan's house for his mom to come home with Kentucky Fried Chicken. She finally arrived, but told them they had to help unload the groceries before they could eat. Cartman encouraged them to all pitch in so as to quicken the job, but he hung back while the other boys went to the garage and methodically pulled the skin off each piece of chicken and devoured it.

When the boys returned with the groceries, they were horrified to see what Cartman had done, but Cartman just up and left, saying he was heading home to "sit on the toilet and read comic books." The next morning at the bus stop, the boys determined to ignore Cartman.
2. What did Cartman see being carried out of his house?

Answer: the old toilet

After the boys thoroughly ignored him at the bus stop, Cartman feared he was dead and ran home. When he got there, he saw men carrying the old toilet away from the house in a box and thought it was a coffin. His fears were only worsened when he overheard the men saying, "Apparently there was so much chicken skin in the system it just ruptured the insides." To top it all off, he then heard his mother moaning from inside the house, which he assumed was in grief over his passing (though she was really just "thanking" the plumber).

Butters told his parents that he'd seen a ghost, but his father wasn't interested:

Mr. Stotch: "That's right. A new form of AIDS which is resistant to drugs. Just one teaspoon of super-AIDS in your butt and you're dead in three years."
Butters: "AAAH! Oh, Jesus!"
Mr. Stotch: "So now you feel better? Ghosts don't exist and there's nothing to be afraid of. Except for super-AIDS."

Cartman had woken Butters up in the middle of the night, trying to get the one person who could "see" him to help figure out why his spirit was trapped on Earth. Cartman told Butters that his spirit couldn't "find the passage to Heaven." Butters asked how Cartman could be sure he was Heaven-bound, specifying, "Well... how do you know you're not supposed to go to... you know...

Heck?" Cartman quickly dismissed the possibility, explaining, "I'm not going to Heck, Butters! I'm not black, all right?"

After Cartman had apologized to Butters, he assumed his soul was a peace and would be going to Heaven. Butters seemed a bit sad, so Cartman tried to comfort him: "Don't be sad, Butters. What awaits each person in Heaven is eternal bliss, divine rest, and $10,000 cash." Unfortunately, Cartman's soul was going nowhere, so he decided he'd have to atone for the wrongs he'd committed in addition to simply apologizing.

Answer: he had a kid's parents killed and then made them into chili which he fed to the kid

Though Cartman did all of those things, his final addition to the list was feeding the kid his dead parents in chili (the kid being hapless Scott Tenorman). Butters was a bit overwhelmed by Cartman's list, saying, "Honestly, I don't know how you're gonna make up for all this."

He also recorded a song about "making it right." The other boys saw him delivering some of the fruit baskets (and repairing Mr. Anderson's fence) but were only annoyed by what they thought were Cartman's shameless and empty ploys to earn forgiveness.

After "fourteen hours" of testing, the doctor proclaimed, "I can say Butters is definitely suffering from aggravated repressed memory syndrome. Butters seemed to be comforted by the thought that Cartman's ghost had really been all in his head, but Cartman showed up at his hospital bed later that night, demanding Butters' help.

As Cartman removed Butters' straps, Butters protested wanly, "Please leave me alone, Eric. My bottom is really sore."

Cartman went into the building first and started making ghost noises, pushing furniture around, and throwing books and papers about. The criminals were so confused by the strange little fat kid that they stood transfixed while Butters snuck in behind them and freed the hostages!

Just then, the Stotches arrived with the doctor, clearly angry that Butters had escaped the Mental Hospital. Knowing what was to come, Butters hung his head, saying, "Oh hamburgers."
